Abstract: Post-bleaching dental sensitivity (DS) following tooth whitening with high-concentration
hydrogen peroxide (HP) is a common adverse effect that can be modulated by adjuvant
therapies, such as ozone therapy, due to its anti-inflammatory and analgesic properties. The
aim of this parallel, blinded randomized clinical trial was to evaluate the whitening efficacy
and dental sensitivity following the use of 35% HP or ozonized gas. Methodology: In the
control group (HP), 35% HP was applied for 45 minutes, preceded by the application of a
desensitizing agent containing 5% Potassium Nitrate and 2% Sodium Fluoride for 10 minutes.
In the ozone group (OZ), ozonized gas at a concentration of 55 µg was applied for 30 minutes
using a custom-made silicone tray. In both groups, the upper arch received the respective
clinical protocols, while the lower arch underwent bleaching with 16% carbamide peroxide
for 14 days. Results: The HP group showed a significant increase in sensitivity over time
across all sessions (p < 0.05), with greater intensity during the intermediate periods of the
procedure and persistence at 24 hours. The OZ group presented values consistently close to
zero, with no significant variation over time. Intergroup differences were observed from the
intermediate time points through 24 hours (p < 0.05), with higher values in the HP group.
Regarding color, both groups showed significant change between baseline and final
assessments (p < 0.05); however, the HP group demonstrated a greater magnitude of
whitening (median ΔC = 7 [7–10]) compared to the OZ group (median ΔC = 1.5 [0–3]) (p <
0.001; r = 0.73). Conclusion: 35% hydrogen peroxide demonstrated superior whitening
efficacy, but was associated with greater dental sensitivity. Ozone therapy alone did not show
a clinically relevant whitening effect, but it resulted in the absence of tooth sensitivity during
and after the procedure.

Abstract: Objective: To evaluate the prevalence of anatomical variations in the mesiobuccal (MB) root
of permanent maxillary first molars and their association with sex, age group, and laterality
using cone-beam computed tomography (CBCT). Methods: A retrospective study was
conducted using CBCT scans obtained from a specialized dental radiology clinic in Cascavel,
Paraná, Brazil, between January 2024 and January 2025. Root morphology was categorized,
and the presence of anatomical variations was assessed. Data were analyzed using descriptive
statistics and inferential tests (Pearson’s chi-square and Fisher’s exact test; α = 5%) with Jamovi
software (v. 2.3.28). Results: A total of 100 teeth were analyzed. The prevalence of the
mesiopalatal (MP) canal was 66%, with a predominance of independent apical foramen. No
statistically significant association was found between anatomical variations and sex. Age
group was significantly associated only with the presence of intercanal communication (p =
0.016), which was more frequent in younger individuals. Laterality showed a significant
association exclusively with the presence of isthmus between teeth 16 and 26 (p = 0.026).
Conclusion: The high anatomical complexity of the MB root reinforces the importance of
CBCT as a complementary tool in endodontic planning, contributing to improved diagnostic
accuracy, treatment predictability, and effective root canal system disinfection.

Abstract: Objective: This study aimed to assess, through a systematic review with network meta-analysis
(NMA), the comparative effects of different desensitizing mechanisms of action on bleaching
induced tooth sensitivity (TS) associated with in-office bleaching in patients with permanent
dentition. (PROSPERO CRD42024544723)
Data: Randomized controlled trials (RCTs) comparing desensitizing agents for the
management of TS associated with in-office bleaching were included. A Bayesian NMA was
conducted to evaluate both the intensity and the risk of TS across different categories of
desensitizing agents, with individual interventions grouped at the network level according to
their mechanisms of action (Tubule-occluder, Nerve-desensitization, Mixed, Association
technique, and Desensitizer-free). TS intensity was assessed using the Visual Analog Scale
(VAS) and Numeric Rating Scale (NRS). Risk of bias was evaluated using the Cochrane Risk
of Bias tool version 2.0 (RoB 2.0), and the certainty of evidence was assessed using the Grading
of Recommendations Assessment, Development and Evaluation (GRADE) approach.
Sources: The literature search was systematically conducted in PubMed, Cochrane Library,
Brazilian Library of Dentistry (BBO), Latin American and Caribbean Health Sciences
Literature (LILACS), Embase, Scopus, Web of Science and grey literature sources.
Study Selection: After duplicate removal and title and abstract screening, 242 studies were
selected for full-text assessment. Of these, 46 studies met the eligibility criteria and were
included in the systematic review.
Conclusion: The findings indicate that the use of desensitizing agents is consistently associated
with a greater reduction in bleaching-induced TS compared with Desensitizer-free in-office
bleaching strategies. However, differences among active desensitizing mechanisms were
limited, imprecise, and did not demonstrate clinically robust superiority of any specific
approach.
Clinical Relevance: These findings support the routine use of desensitizing agents to reduce TS
associated with in-office bleaching. Given the absence of consistent clinically meaningful
differences among desensitizing mechanisms, the choice of desensitization strategy may be guided by clinical practicality, availability, and patient-related factors rather than presumed
mechanistic superiority.

Abstract: Research question: Does the angulation between mandibular implants influence the need for
prosthetic maintenance in O-ring–retained overdentures, and does it affect patients’ quality of
life and perception? Introduction: The influence of interimplant angulation on the prosthetic
maintenance of O-ring–retained mandibular overdentures, as well as its potential impact on
quality of life and patient perception, remains unclear in long-term clinical studies. Objective:
To evaluate whether interimplant angulation influences prosthetic maintenance in O-ring–
retained mandibular overdentures and to analyze its relationship with patients’ quality of life
and self-perception. Materials and Methods: This longitudinal retrospective study (2012–
2025) analyzed records of patients rehabilitated with mandibular overdentures retained by two
implants (n = 35). Interimplant angulation was measured radiographically and analyzed as a
continuous variable. The number of O-ring replacements and prosthetic adjustments during
follow-up were recorded. Quality of life and patient perception were assessed using the OHIP-
EDENT and SATO instruments in a chairside sub-sample (n = 20). Associations among
variables were analyzed using non-parametric tests, specifically Spearman’s correlation
coefficient, with a significance level of 5%. Results: The most frequent prosthetic maintenance
event was O-ring replacement, with an approximate mean frequency between 0.5 and 1.0
replacement per year of use. Statistical analysis showed no significant association between
implant angulation and the number of O-ring replacements or the need for prosthetic
adjustments (weak correlation; |ρ| < 0.30; p > 0.05). In the evaluated sub-sample, prosthetic
satisfaction scores (SATO) were high, predominantly ≥ 85, while quality-of-life scores (OHIP-
EDENT) indicated low negative impact, with no significant correlation with the degree of
implant angulation (p > 0.05). Conclusion: O-ring–retained mandibular overdentures
demonstrated long-term functional stability, predictable maintenance needs, and no significant
influence of implant angulation on clinical performance over the evaluated period. Clinical
Implications: These findings support O-ring–retained mandibular overdentures as a reliable
therapeutic option, even in the presence of small angular variations between implants, and
provide practical guidance for treatment planning, clinical execution, and patient counseling
regarding longevity and maintenance.
