Abstract: Objective: To analyze the morbidity and mortality of invasive diseases caused by Streptococcus pyogenes in a trinational border municipality, estimating epidemiological indicators of hospitalization and lethality, characterizing the sociodemographic and clinical profile of the cases, and identifying factors associated with death. Methods: An epidemiological, descriptive, and cross-sectional study was conducted with 41 cases of invasive Streptococcus pyogenes diseases reported in Foz do Iguaçu, Paraná, Brazil, between 2023 and 2025. Sociodemographic and&#xD;
clinical characteristics and patient outcomes were analyzed using descriptive statistics, Chi-square test and multiple&#xD;
logistic regression. Results: Among the 41 cases analyzed, there was a predominance of females (56.1%), adults&#xD;
(53.7%) and residents of Brazil (92.7%). Skin and soft tissue infections accounted for the main infectious focus (39.0%), followed by pulmonary infections (26.8%). Among the patients, 26.8% had comorbidities and 29.3% died. The most frequent signs and symptoms were fever (81.8%), pain (48.5%), hyperemia/purpura/rash (45.5%) and edema (45.5%). The most used antimicrobials were clindamycin (59.4%) and ceftriaxone (50.0%). Lethality remained high and stable during the period analyzed, ranging from 27.3% to 30.4%, with no statistically significant difference between the years (p=0.9812). The logistic regression model did not identify a statistically significant association between death and the sociodemographic or clinical variables evaluated. Conclusions: Invasive diseases caused by S. pyogenes showed high lethality in the city studied, with a predominance of cases in adults, women, and individuals with skin and soft tissue involvement. Although no factors associated with death were identified, the results highlight the epidemiological relevance of these infections in the triple border region and reinforce the need to strengthen epidemiological surveillance, laboratory diagnostic capacity, and early recognition of severe cases.

Abstract: Respiratory diseases that affect the upper and lower respiratory tract are an important cause of morbidity and mortality, being caused by viral agents such as influenza virus, respiratory syncytial virus, adenovirus, hantavirus and coronavirus. Severe Acute Respiratory Syndrome (SARS) stands out in this context due to the high frequency of hospitalizations and deaths, especially among vulnerable groups. This study aimed to describe the epidemiological profile of respiratory viruses associated with SARS and to analyze factors associated with death in cases notified by the Padre Germano Lauck Municipal Hospital (HMPGL), located in the triple international border region, from 2016 to 2023. This is an observational, descriptive and analytical study, with a quantitative approach and the use of retrospective data from hospital epidemiological surveillance records. A total of 3,747 complete SARS records with laboratory confirmation for COVID-19, influenza, or other respiratory viruses and with information on clinical evolution were included. The variables analyzed included gender, age group, presence of comorbidities, use of Oseltamivir, vaccination, admission to the intensive care unit (ICU), and outcome (discharge or death). Descriptive analysis and multivariate logistic regression were performed to identify factors associated with death, stratifying by etiological agent. There was a predominance of COVID-19 cases (61%), followed by other respiratory viruses (33.4%) and influenza (6.8%). The overall death rate was 28.4%. Advanced age, male gender, higher number of comorbidities, and ICU admission were significantly associated with increased risk of death, especially among COVID-19 and influenza cases. Vaccination against COVID-19 showed a protective effect, reducing the probability of death. Among influenza cases, the use of oseltamivir was associated with ICU admission, suggesting its predominant use in contexts of greater clinical severity. We conclude that SARS remains associated with high hospital morbidity and mortality, with consistent clinical determinants among different viral agents. The findings reinforce the importance of hospital epidemiological surveillance, the expansion of vaccination strategies, and the improvement of the quality-of-care records, especially in international border regions.; Las enfermedades respiratorias que afectan al tracto respiratorio superior e inferior son una causa importante de morbilidad e mortalidad, siendo causadas por agentes virales como el virus de la gripe, el virus respiratorio sincicial, el adenovirus, el hantavirus y el coronavirus. El Síndrome Respiratorio Agudo Severo (SARS) destaca en este contexto debido a la alta frecuencia de hospitalizaciones y muertes, especialmente entre los grupos vulnerables. Este estudio tuvo como objetivo describir el perfil epidemiológico de los virus respiratorios asociados al SARS y analizar factores asociados a la muerte en casos notificados por el Hospital Municipal Padre Germano Lauck (HMPGL), ubicado en la región fronteriza internacional triple, desde 2016 hasta 2023. Se trata de un estudio observacional, descriptivo y analítico, con un enfoque cuantitativo y el uso de datos retrospectivos de registros de vigilancia epidemiológica hospitalaria. Se incluyeron un total de 3.747 registros completos de SARS con confirmación de laboratorio para COVID-19, gripe u otros virus respiratorios, y con información sobre la evolución clínica. Las variables analizadas incluyeron género, grupo de edad, presencia de comorbilidades, uso de Oseltamivir, vacunación, ingreso en la unidad de cuidados intensivos (UCI) y resultado (alta o fallecimiento). Se realizaron análisis descriptivos y regresión logística multivariante para identificar factores asociados a la muerte, estratificando por agente etiológico. Predominaron los casos de COVID-19 (61%), seguidos de otros virus respiratorios (33,4%) y la gripe (6,8%). La tasa global de mortalidad fue del 28,4%. La edad avanzada, el sexo masculino, un mayor número de comorbilidades y el ingreso en UCI se asociaron significativamente con un mayor riesgo de muerte, especialmente entre los casos de COVID-19 y gripe. La vacunación contra la COVID-19 mostró un efecto protector, reduciendo la probabilidad de muerte. Entre los casos de gripe, el uso de oseltamivir se asoció con el ingreso en UCI, lo que sugiere su uso predominante en contextos de mayor severidad clínica. Concluimos que el SARS sigue asociado a una alta morbilidad y mortalidad hospitalaria, con determinantes clínicos consistentes entre los diferentes agentes virales. Los hallazgos refuerzan la importancia de la vigilancia epidemiológica hospitalaria, la expansión de las estrategias de vacunación y la mejora de la calidad de los registros de atención, especialmente en las regiones fronterizas internacionales.

Abstract: Thrombophilia, a condition that predisposes to the formation of blood clots, is more relevant during pregnancy due to the physiological changes that favor the appearance of thrombi. Pregnant women with thrombophilia are at high risk of complications, such as recurrent miscarriages, fetal growth restriction, preeclampsia, and venous thromboembolism. In Brazil, the Unified Health System (SUS) offers protocols for the diagnosis and treatment of these conditions, but access to treatment with anticoagulants, such as enoxaparin sodium, is limited to clinical criteria and diagnosis. In border regions, such as Foz do Iguaçu, the situation is aggravated by the difficulty of caring for pregnant women in situations of social vulnerability, many of whom face challenges in meeting the criteria of the clinical protocol, which hinders access to adequate diagnosis and treatment. This study aimed to describe the therapeutic itinerary (TR) of pregnant women who need treatment with anticoagulants in a border region, analyzing the challenges faced by these women and the possible consequences of this underdiagnosis in the context of a border municipality. This is qualitative, exploratory and descriptive research, based on content analysis, according to Minayo, and conducted according to the criteria of the Consolidated Criteria for Reporting Qualitative Research (COREQ). The theoretical-methodological approach adopted was hermeneutics-dialectics. The study included 19 women who used anticoagulants during pregnancy, from 2015 to 2025, selected by nonprobabilistic snowball sampling. The data were produced through semi-structured interviews and analyzed up to the point of theoretical saturation. The results revealed therapeutic itineraries marked by the naturalization of pregnancy loss, fragmentation of care, lack of longitudinally and difficulties in accessing the diagnosis of thrombophilia. Institutional and financial barriers to performing laboratory tests were evidenced, as well as obstacles to timely access to enoxaparin sodium, often obtained through judicialization. The specificities of the border territory intensified these difficulties, especially for women in situations of greater vulnerability. The PCDT, although it regulates care, has limitations in responding to the needs of pregnant women with clinical conditions compatible with thrombophilia, especially in border contexts. The study points to the need to review care flows, strengthen primary care, and formulate public policies that expand access to diagnosis and treatment, contributing to the reduction of avoidable pregnancy losses and to the qualification of maternal and childcare.; La trombofilia, una condición que predispone a la formación de coágulos sanguíneos, es más relevante durante el embarazo debido a los cambios fisiológicos que favorecen la aparición de trombos. Las mujeres embarazadas con trombofilia tienen un alto riesgo de complicaciones, como abortos recurrentes, restricción del crecimiento fetal, preeclampsia y tromboembolismo&#xD;
venoso. En Brasil, el Sistema Unificado de Salud (SUS) ofrece protocolos para el diagnóstico y tratamiento de estas afecciones, pero el acceso al tratamiento con anticoagulantes, como la enoxaparina sódica, se limita a criterios clínicos y diagnóstico. En regiones fronterizas, como Foz do Iguaçu, la situación se agrava por la dificultad de atender a mujeres embarazadas en situaciones de vulnerabilidad social, muchas de las cuales enfrentan dificultades para cumplir con los criterios del protocolo clínico, lo que dificulta el acceso a un diagnóstico y tratamiento adecuados. Este estudio tuvo como objetivo describir el itinerario terapéutico (TR) de las mujeres embarazadas que necesitan tratamiento con anticoagulantes en una región fronteriza, analizando los desafíos que enfrentan estas mujeres y las posibles consecuencias de este subdiagnóstico en el contexto de un municipio fronterizo. Se trata de una investigación cualitativa, exploratoria y descriptiva, basada en el análisis de contenidos, según Minayo, y realizada según los criterios de los Criterios Consolidados para la Investigación Cualitativa (COREQ). El enfoque teórico-metodológico adoptado fue hermenéutica-dialéctica. El estudio incluyó a 19 mujeres que usaron anticoagulantes durante el embarazo, de 2015 a 2025, seleccionadas mediante muestreo no probabilístico en bola de nieve. Los datos se produjeron mediante entrevistas semiestructuradas y se analizaron hasta el punto de saturación teórica. Los resultados revelaron itinerarios terapéuticos marcados por la naturalización de la pérdida del embarazo, fragmentación de la atención, falta de longitudinalidad y dificultades para acceder al diagnóstico de trombofilia. Se evidenciaron barreras institucionales y financieras para realizar pruebas de laboratorio, así como obstáculos para el acceso oportuno a la enoxaparina sodio, a menudo obtenidos mediante judicialización. Las especificidades del territorio fronterizo&#xD;
intensificaron estas dificultades, especialmente para mujeres en situaciones de mayor vulnerabilidad. El PCDT, aunque regula la atención, tiene limitaciones para responder a las necesidades de mujeres embarazadas con condiciones clínicas compatibles con la trombofilia, especialmente en contextos fronterizos. El estudio señala la necesidad de revisar los flujos de&#xD;
atención, fortalecer la atención primaria y formular políticas públicas que amplíen el acceso al diagnóstico y tratamiento, contribuyendo a la reducción de pérdidas de embarazo evitables y a la cualificación del cuidado materno e infantil.

Abstract: Population aging in Brazil poses significant challenges to public health, especially regarding the loss of functional capacity and increased susceptibility to falls and fractures, events that severely compromise the autonomy and dignity of the elderly. This research aimed to describe the epidemiological profile and identify predictors of mortality among elderly individuals, aged 60 years or older, treated for fractures at a public hospital in the municipality of Foz do Iguaçu, a tri-border region (Brazil-Paraguay-Argentina). Methodologically, a cross-sectional, retrospective, and quantitative study was conducted, analyzing secondary data from 2,586 treatments that occurred between 2018 and 2024. In the data analysis, descriptive statistics and&#xD;
binary logistic regression were applied to determine the association between clinical variables and the outcome of death, with a significance level of α=0.05. The results revealed a predominance of women (61.9%), the 60-80 age range (74.5%), and of Brazilian nationality (94.4%), although the geographical location recorded care for foreigners, especially Paraguayans. Femur (36.9%) and forearm (31.5%) fractures were the most prevalent, with falls from standing height being the main mechanism of trauma. The in-hospital mortality rate was 1.3%. The most robust predictors of mortality identified were nephropathy (OR=24.20), ICU bed occupancy (OR=14.60), COPD/pulmonary emphysema (OR=7.72), and heart disease (OR=6.79). Post-fracture mortality has been shown to be multifactorial, dependent on reduced physiological reserve and comorbidity burden, suggesting that renal dysfunction acts as a critical marker of extreme frailty. It is concluded that orthopedic trauma in the elderly is a systemic event that requires interdisciplinary management and a humanized approach to the aging process. As a practical proposal, the implementation of a risk protocol for elderly nephropathic patients and the strengthening of primary care in the prevention of domestic falls are recommended, aiming to guarantee safe, dignified, and equitable aging in a border region.; El envejecimiento de la población en Brasil impone desafíos significativos a la salud pública, especialmente en lo que respecta a la pérdida de capacidad funcional y la mayor susceptibilidad a caídas y fracturas, eventos que comprometen gravemente la autonomía y la dignidad de las personas mayores. Esta investigación tuvo como objetivo describir el perfil epidemiológico e identificar los predictores de mortalidad de personas mayores de 60 años tratadas por fracturas en un hospital público del municipio de Foz do Iguaçu, en la región trifronteriza (Brasil-Paraguay-Argentina). Metodológicamente, se llevó a cabo un estudio transversal, retrospectivo y cuantitativo, en el que se analizaron datos secundarios de 2.586 visitas realizadas entre 2018 y 2024. En el análisis de datos, se aplicaron estadísticas descriptivas y regresión logística binaria para determinar la asociación entre variables clínicas y el resultado de la muerte, con un nivel de significación α=0,05. Los resultados revelaron una predominancia de mujeres (61,9%), de 60 a 80 años (74,5%) y de nacionalidad brasileña (94,4%), aunque la ubicación geográfica registró atención para extranjeros, especialmente paraguayos. Las fracturas femorales (36,9%) y de antebrazo (31,5%) fueron las más prevalentes, siendo las caídas desde el propio nivel como principal mecanismo de trauma. La tasa de mortalidad hospitalaria fue del 1,3%. Los predictores de mortalidad más robustos identificados fueron nefropatía (OR=24,20), ocupación de camas en UCI (OR=14,60), EPOC/enfisema pulmonar (OR=7,72) y enfermedades cardíacas (OR=6,79). Se ha verificado que la mortalidad post fractura es multifactorial, dependiente de la reducción de la reserva fisiológica y la carga de comorbilidades, lo que sugiere que la disfunción renal actúa como un marcador crítico de fragilidad extrema. Se concluye que el trauma ortopédico en personas mayores es un evento sistémico que requiere una gestión interdisciplinar y una visión humanizada del proceso de senescencia. Como propuesta práctica, se recomienda la implementación de un protocolo de riesgo para la nefropatía en ancianos y el fortalecimiento de la atención primaria en la prevención de caídas domésticas, con el objetivo de garantizar un envejecimiento seguro, digno y equitativo en el territorio fronterizo.
